Obelisk /case & /case_reason
Every moderation action has a case number. Look one up, or correct its reason.

Who can use them: Server owner, Administrator, Manage Server, or a member or role granted the mod_cases permit., plus anyone with another moderation permit or the matching native Discord permission (Ban Members, Kick Members or Moderate Members).
